Ontario Auditor General: 60% of Approved Medical AI Scribes Recorded Wrong Drug, Nine Hallucinated Clinical Details
Ontario's auditor general found that 9 of 20 government-approved AI medical transcription systems fabricated clinical information including diagnoses and treatment suggestions never discussed during patient encounters. Sixty percent of approved scribes recorded a different drug than what was actually prescribed. Accuracy accounted for only 4% of procurement scoring while 'domestic presence in Ontario' was weighted highest at 30%, and 11 of 20 approved vendors submitted no third-party audit reports. Approximately 5,000 Ontario physicians currently use these systems.
