Claude Code v2.1.178 Adds Tool(param:value) Permission Syntax and Classifier-Gated Subagent Spawns

The June 15 release introduces parameter-matching permission rules with wildcards, e.g. `Agent(model:opus)` to block Opus subagents, plus nested `.claude/skills` directories that auto-load by working directory (shown as `<dir>:<name>` on clashes). Improved auto mode now runs the classifier on subagent spawns *before* launch, closing a gap where a subagent could request a blocked action without review. Also fixes `--fallback-model` being ignored in compaction and MCP server-level specs ignored in subagent `disallowedTools`.
