Claude Code 2.1.208 Ships CLAUDE.md-Trimming /doctor Check, Auto Draft-PR Background Agents, and an Opus-Capped Explore Agent

The July 14 Claude Code release (v2.1.208, with a 2.1.209 follow-up fixing dialogs in background sessions) adds a `/doctor` check that proposes trimming checked-in CLAUDE.md content Claude could derive from the codebase, makes background agents commit/push and open a draft PR on completion instead of stopping to ask, and upgrades the built-in Explore agent to inherit the session model (capped at opus) rather than running on haiku. Screen-reader mode and streaming/MCP fixes also landed. These are distinct from the July 11 enterprise-controls update and are immediately useful to daily Claude Code users.
