Claude Code `/usage` Was Overattributing Token Spend to MCP Servers

2.1.222 fixes `/usage` counting every turn after any call to an MCP server as that server's usage; a server's share now reflects only the requests that actually consumed its tool results. If you have been pruning MCP servers based on `/usage` attribution, your data was inflated — re-measure before removing anything. The same release also fixes org-restricted `model: opus`-style subagent aliases silently dropping to the parent model instead of stepping down to the newest org-allowed model in that family.
