Claude Code 2.1.235, out 2026-08-18, fixes whole-prompt-cache invalidation triggered when a language server disconnected mid-session. A dropped LSP would quietly blow the entire cache, so the next turn re-paid full input cost with nothing on screen to explain the jump. If you have been seeing unexplained token spikes in long sessions on projects with a flaky language server, upgrading is the fix.
