Claude Code 2.1.258 fixes a launch regression that broke macOS Monterey for three versions

Released 2026-09-01, v2.1.258 fixes Claude Code failing to launch entirely on macOS 12 Monterey, a regression introduced in 2.1.255 and therefore live across 2.1.255 through 2.1.257. It also fixes remote and scheduled sessions failing with "user messages must have non-empty content" after a re-sent permission approval could not be applied, which is the kind of break that only shows up in unattended scheduled runs.
