A v2.1.261 fix stops in-process agent-team teammates from re-sending their first-turn tool and skill announcements on the second turn. Because those announcements sit in the request prefix, repeating them changed the prefix and blew the cache on every teammate's second turn. Anyone running multi-agent teams on a subscription or metered plan was paying full input price for turn two across every teammate without any visible symptom.
