Pattern: identity and account boundaries are now a named class of coding-agent bug
Codex 0.155.0 devotes a block of fixes to it — switching accounts invalidates remote-control sessions, cached WebSocket state and model catalogs belonging to the previous identity (#43906, #44341, #44489), model catalog caches are scoped to provider and auth identity (#43897), MCP verification is workspace-scoped (#43524), and staged enterprise OIDC login with coordinated logout lands (#43844). Claude Code 2.1.275 shows the same shape from the other side: gateway sign-in now names the signed-in account for confirmation before saving the credential, `/status` displays it, and `/logout` ends the session on gateways advertising token revocation. Anyone running more than one account against one CLI has been carrying stale cached state across the switch, which matches the artifact-404-across-accounts behaviour already seen locally.
