With cooperative patients the models were correct 350/350 times; with resistant patients presenting identical medical facts but a reluctant attitude, accuracy fell to 225/350 (64%), and to 22% in severe cases. The failure mode is sycophancy with a measurable clinical cost: the models substituted lifestyle tips for referrals when users downplayed symptoms.
